For your convenience we have provided you with the intake forms. If you would like, please fill out the appropriate forms and bring them in with you. For those who would like to fill these forms out in the office, please arrive 10 minutes early.





What to expect:

 

The first visit is always the longest, typically about 45 minutes.  This is so the doctors can gather and assess important information about your health history, perform functional and orthopedic examinations, perform postural analysis and treat your complaints.  Subsequent visits are generally limited to 15-20 minutes so you can get back to your busy life.





What to bring:

 

• Filled out New Patient form (if you chose to print it)

• Insurance card/policy information

• Recent diagnostic imaging and lab results

• Wear comfortable clothing if possible
